Five Wishes - The Gift of Conversation

  • Jesup Memorial Library 34 Mt. Desert Street Bar Harbor, Maine 04609 (map)

Many individuals recognize the importance of discussing their end-of-life care wishes, yet few have had these conversations. Have you had that talk? In this session, you will discover more about advanced care planning and how to share this vital conversation with those who matter most in your life. The emphasis will be on:

  • How to initiate advanced care planning discussions with loved ones.

  • Choosing the right person to speak for you when you cannot communicate for yourself due to a health crisis.

  • The significance of updating your documents as life situations change.

All attendees will receive a complimentary Five Wishes document, which will guide you through the necessary steps and considerations. Once completed and signed, this document serves as a legally valid advance directive. Join us to ensure that your end-of-life wishes are honored. This class will be led by Cookie Horner, a Five Wishes Certified Presenter.

This class will be held in-person only. Registration is encouraged.
